Maybe Snape says or does something to Regulus, which (as much as Sirius dislikes his family) stings Sirius’ family feeling which he won’t admit he still has. If he’s going to do something irrational and dangerous on a sudden jag, that messy relationship that he’s not very rational about to begin with is well placed to be part of it. Meanwhile, we know he has extremely complicated, not entirely reasoned-out responses to and feelings about his family, particularly after the age of about fifteen where he runs away/is kicked out. This is private and personal, which is what makes it so dangerous. If he remembered they existed, he would remember how much his actions could harm them, so he needs to be so consumed that he forgets. It needs to be something he broods on and puts into motion entirely by himself, without thinking about his friends as involved or affected in any way. If it was retaliation for something that involved his friends, then his friends would understand why he wanted revenge, even if the extent surprised them, and in context Sirius’ actions need to be truly, sincerely shocking. Remus and James can’t know what he’s thinking of doing to retaliate until after he does it, because the werewolf incident can easily get Remus killed and we know that James is appalled (to the point where he goes and drags Severus back) as soon as he understands what Sirius has done. It can’t be anything concerning his friends.
